This Telehealth Informed Consent ("Consent") describes telehealth services delivered by the independent, licensed clinicians who participate in The Network Health's provider network. Please read it carefully before receiving care.
The Network Health's role. The Network Health LLC is an administrative and network services organization. It does not practice medicine or provide medical care. Your care is provided by an independent, licensed clinician who exercises their own professional judgment. Where you receive care through a partner platform, that platform may present its own consent as part of intake; this document explains the telehealth model generally.
Telehealth is the use of secure electronic communications — such as online intake, messaging, audio, and video — to deliver clinical care remotely, without an in-person office visit. Care may be provided asynchronously (you submit information and a clinician reviews and responds) or synchronously (a real-time visit).
Clinical services are provided by licensed clinicians who are responsible for your care and who exercise independent professional judgment. The Network Health supports these clinicians with administrative and network services only and does not direct clinical decision-making.
As with any medical service, telehealth has limitations and risks, including:
Your clinician will let you know if your condition is not appropriate for telehealth and will recommend in-person care when needed.
A clinician will prescribe medication only when they determine, in their independent professional judgment, that it is clinically appropriate and consistent with applicable law. A visit does not guarantee that any medication will be prescribed. Certain medications, including controlled substances, may be restricted or unavailable through telehealth.
No specific result or outcome is promised or guaranteed. Medicine is not an exact science, and reasonable clinicians may reach different conclusions.
Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use telehealth for urgent or emergency conditions.
Your health information is protected under applicable privacy laws, including HIPAA where it applies. Information may be shared among your treating clinician and the personnel and systems supporting your care, as needed to provide services and as permitted or required by law. By proceeding with a telehealth visit, you acknowledge that you have read and understood this Consent, that you have had the opportunity to ask questions, and that you voluntarily consent to receive care via telehealth from an independent, licensed clinician under the terms described above.
